Surgical guides for dental implants

What are surgical guides for dental implant placement?

Nowadays, surgical guides for dental implants offer a technological element that helps the specialist to plan and execute your treatment in a much more precise and reliable way. These are made by computer after accurately planning the digital placement of your implants.

A surgical guide is a surgical element, which allows the specialist to place the implants according to a previous computerized study and planning, in a careful and precise way.

Thanks to this innovative tool, surgical time is reduced and a less painful and less swollen post-operative period is achieved.

From this procedure, an innovative 3D digital technology of the intraoral scanner is combined with the 3D study of the CT-CBCT (computed axial tomography – cone beam computed tomography), the digital design of the guide and the high definition printing with 3D printer; to obtain a 100% customized and precise guide.

For this reason, a surgical guide:

Ensure that the implant angulation is correct.

Controls the distance of the implant from other implants.

Establishes placement in front of the dental roots, avoiding derived problems.

What types of surgical guides are there?

First of all, these biomedical devices are indispensable for the proper insertion of dental implants. Several types of guidelines are currently available: restrictive, semi-restrictive and non-restrictive.

Depending on the type of guide required, the diameter of your cylinders may vary, as titanium tubes will be inserted. In the same way, surgical guides in implantology also differ according to the type of support: tooth, mucosa or bone.

Restrictive surgical guides

These guides limit the vestibular or lingual positions and have depth stops, which leads to accurate preparations according to the planning.

On the other hand, a semi-restrictive guide incorporates a guide tube that corresponds to the initial part of the preparation; however, the surgeon must continue freehand with the surgical protocol, which can also cause complications.

In this case, the guide indicates to the surgeon the position of the implants in relation to the prosthesis. It prevents greater control over the direction or depth of drilling, which may cause angulation errors, lack of parallelism or compromise of anatomical structures.

Surgical guides supported by teeth

They are undoubtedly the most suitable for edentulism and edentulous cases with a free end less than 30mm from the edentulous area. They are also used for flap and non-flap surgeries.

It is, if desired, the most accurate surgical guide, especially in cases of tooth-bound edentulism.

Stereolithographic surgical guides with mucosal support

These surgical guides for dental implants are recommended for completely edentulous patients.

Obviously, its main advantage lies in the minimally invasive or flapless surgical technique that avoids the need for mucoperiosteal flaps.

However, it requires a certain degree of expertise to operate it.

In particular, this surgical guide should be used with an occlusion key, after which a retaining pin should be placed to keep it firmly in place with maximum stability.

The support surface on which the guidewire sits is the mucosa, which is a compressible surface between 3 to 4 mm thick. Unequal pressure can cause it to tip over and create a deviation in the initial point of impact and, therefore, in the patient’s planned angulation.

Bone-supported surgical guides

This type of surgical guide requires considerable elevation of the mucoperiosteal (full-thickness) flap to place and stabilize it.

As a result, specialists are using them less and less, and some have even abandoned them, as they contradict the philosophy of guided surgery, which aims to promote mini-invasive techniques.

Surgical guides in implantology

Surgical guides are the latest advance in dental implant technology, which makes it possible to integrate impressions into an intraoral scan of the desired surgical implant site with a computer-guided 3D implant planning system.

Similarly, surgical guides in implant dentistry reproduce the exact surfaces of the patient’s intraoral environment and help the surgeon to drill implant sites in the bone with optimal precision.

Once placed in the patient’s jaw, the surgical guide uses sleeves to help guide the surgical instruments and implant to the proper location.

Phases of dental implantology

We will list the different phases for the placement of dental implants:

Study and treatment plan. At this stage the patient is examined by a dentist specialized in implants to know the condition of the mouth, teeth, tissues and bone. Later, the dentist carries out various diagnostic tests (photos, study models, radiographic tests...) and a cone beam computed tomography, a definitive diagnostic test that provides us with a three-dimensional image of the space where the implants will be placed, as well as the quality of the bone where they will be fixed.

Intervention and placement of dental implants. Within this section, we have to differentiate 3 stages which are: first surgical phase, healing period and second surgical phase.

Placement of the definitive prosthesis. Finally, it can take between 15 days or two months from the time the measurements are taken or scanned to the definitive placement of the prosthesis or crowns. During this time and depending on the type of prosthesis, adjustment tests are performed to ensure that the connection with the implant is adequate and that the esthetic finish is in harmony with the rest of the patient's teeth. After placing the definitive prosthesis, a review is scheduled after 6 months to know the evolution of the patient.

Annual implantology revisions. In the same way, a series of revisions and maintenance must be carried out every 12 months. It is from these simple and routine, but essentially safe, processes that the long-term treatment is defined; therefore, they will require a certain constancy in the management of the patient, who will have to visit the dentist periodically. If the patient does not attend annual check-ups, the chances of implant failure increase.

Advantages and disadvantages of dental implants

Among its main advantages, it is worth mentioning:

The prostheses and bridges that have been placed on the implants do not slip in the mouth; therefore, there is no impediment to chewing and speech, i.e., the functionality of the mouth is fully restored.

They have a low percentage of rejection, since biocompatible materials are used. The highest percentage of rejections occurs in patients who smoke. On the other hand, its duration is between 10 and 20 years, depending on the patient's habits and periodic check-ups.

It increases self-esteem, improves aesthetics, patient confidence and quality of life.

It avoids bone loss because the false tooth root (i.e. the implant) stimulates the bone of the jaws.

On the other hand, its disadvantages are:

The cost of treatment is higher than for prostheses.

He needs surgery and several visits to the dentist. Most of the complications can be mild, such as swelling or bleeding, which can be resolved with proper follow-up.

The maxillary bones must be in good bony condition to be able to carry out the implant.
